Chronic Disease Management

Comprehensive care. We can manage all chronic disease states on site. Care needs are catered to each individual, and their personal goals. Family involvement is encouraged. Visits can be auto-scheduled monthly. This includes medication management, continuous refills, on-site diagnostics (labs, urine analysis, ultrasound and x-ray), and proper coordination with Home Health and other specialty providers.
Acute Care and Follow-up Visits

Immediate scheduling can be arranged for changes in condition, illness, or injury (e.g., viral illness, falls with injuries, behavioral changes).

Routine management of chronic conditions is ongoing and intended to preserve health and functionality. This includes resource management and additional community referrals as appropriate.
Telemedicine Visits

Remote Telemedicine services can be requested or recommended as needed. Ideal for more urgent needs, and/or to establish care prior to moving into an assisted living facility. All requested form submissions are included.
Geri-Psych Services

We offer specialized care that fully addresses our seniors' psychological needs. We can formulate tailored care plans with purposeful reassessments and medication adjustments. This is particularly helpful for geriatric patients with behavioral symptoms related to dementia.
Post Hospitalization Care

Hospitalizations require fluid follow up and coordination of care. We offer transitional visits for patients discharging from a hospital or rehab. These visits include a comprehensive review of hospital and rehab records, guidance with follow-up specialty referrals, collaboration with Home Health Care, and a complete medication reconciliation.
Palliative Care Services

We can offer tailored support when the hospital system is not helping.
Palliative care is catered towards improved quality of life, better symptom management, and enhanced emotional/spiritual well-being. This includes support for caregivers, helping them navigate the challenges of aging & make informed decisions based on their own personal goals of care.
